HealthShare NSW, on behalf of Central Coast Local Health District (CCLHD) is inviting tenders for Care Awaiting Placement Program (CAPP).

The Services to be provided in accordance with the Proposed Contract includes funded access to temporary bed placement (in facilities across CCLHD) to accommodate patients who are suitable for discharge from acute care services but require ongoing support until long term care arrangements are finalised. Such cohort of patients could include patients requiring permanent RACF placement, require respite or palliative care, or awaiting home modification completion. The Services are required as part of the Central Coast Local Health Districts winter strategy and to improve overall patient flow and acute bed availability during peak capacity periods.
