Tender Details

Provision of palliative care home support comprising;

  • Registered Nurse (RN) - 6 hour evening service shift (100% of patients)
  • The RN will visit clients who are in deteriorating and terminal phases
  • The RN to provide clinical care
  • The RN will receive clinical referral notes and clinical handover from SWSLHD service team
  • access to SWSLHD Medical After Hours on call palliative care medical specialist
  • provision of services for End of Life Care
  • Optional day time care worker to provide personal care/some respite (60% of patients)
  • Co-ordination for the intake to the Program via the SWSLHD Triple I (Hub)
  • Review of new referrals
  • Ensuring home visit risk and safety is addressed
  • Planning of home visits
  • Recording of data, completion of progress notes and escalation of clinical needs
  • Provision of a daily list detailing client status
  • Maximum 10 clients serviced at any one time (5 in each Hub)
  • Maximum of 7 days of services delivered to each client, reviewed for possible 7 day extension
  • Total maximum service provision of 14 days for each client, reviewed for possible further extension
  • Services to be provided by appropriately qualified personnel
  • Attendance at operational and contractual meetings

  • Demonstrated capacity to meet the service requirements detailed above, including details of qualified personnel
  • Demonstrated capacity to service either, or both, of the LGA Hubs
