Southern NSW Local Health District (SNSWLHD) is seeking to engage a Non-Government Organisation (NGO) to provide Home Based Outreach Services (HBOS) within Queanbeyan/Cooma, Goulburn/Yass Valley and Bega Valley Shire Local Government Areas (LGA) to improve outcomes for people with a mental illness through support in the development of personal life pathways.
The Home Based Outreach Service (HBOS) is a program designed to deliver community based psychosocial support for people with a mental illness in the Queanbeyan/Cooma , Goulburn/Yass Valley and Bega Valley Shire Local Government Areas (LGA). The program is designed to enhance the existing State Wide Housing & Accommodation Support Initiative (HASI).
The workforce for this program will be made up of staff from SNSWLHD and staff from the successful tenderer. The focus of the partnership is to work together in supporting people with a mental illness by providing improved access to community social, leisure, recreational opportunities as well as vocational, education, training & employment services.
