Tender Details
The Services to be performed in accordance with the Proposed Human Services Agreement to provide non-clinical services to eligible older people participating in the Transitional Aged Care Programme (TACP). TACP provides short-term restorative care to optimise the function and independence of older people after a hospital stay. The programme is goal orientated and therapy focussed and runs for up to 12 weeks after discharge from hospital. The Services are described in Part B: Specifications.
