NSW Health / For Goods and Services relating to health coaching and self management for NSW Health Severe Chronic Disease Management Program. - HAC 10/08

The purpose of this Tender is to establish a panel contract for the supply of goods and services relating to health coaching and self management for NSW Health Severe Chronic Disease Management Program.

The scope of this Tender covers five separate but aligned components which together provide a range of goods and services required to implement the SCDM program.

·                The five components to the panel are:

·                Web-enabled health coaching systems: for NSW Health or Health Services to use under licence as required

·                Chronic Disease Management/ Self-management coaching services: for NSW Health or Health Services to outsource services as required.

·                Training in health self-management support: including Train the Trainer so NSW Health, Health Services or Divisions of General Practice can procure services as required. Training is to be accessible for clinicians and GPs.

·                Awareness training of Self Management Support for NSW Health clinical staff.

·                Remote telemonitoring goods and services.
