Professional service to use participatory action research to facilitate the Transition from current traditional and standardised service delivery models to Individualised (Packaged) Support. The Strategy is to occur in NSW over three years. The service is to build participatory action research capabilities and capacities of DADHC staff and systems to sustain the Transition and to generate evidence of the value of Individualised (Packaged) Support in the NSW context. This research will set an international benchmark for longitudinal research into personalised approaches to support for people living with disability.
